They are the first and to this day they are the best.  At the onset of my career in the field of electrolysis, I was an R.A. Fischer distributor and the first electrologist to purchase and take their manufactures training courses on the computerized epilator in 1985, 1986 and 1987. Understanding the unique needs of the Trans population and backed with unprecedented training for the use of equipment of the highest standard, I, Kimberly Williams, R.E., Dean, championed and pioneered the practical applications of computerized-electrology-electrolysis-blend by developing a custom programming using Computerized Short-Wave-Galvanic-Blend.

Of major importance I would like every woman contemplating permanent hair removal by electrolysis to be aware of that even though this technique appears to be for Transsexual patients only it is a matter of fact that I treat all patients with severe unwanted facial hair problems with the same technique. Furthermore my trade secrets and applications used from one patient to another are individualized for each and every patient unique need to achieve success and permanent hair removal no matter what your identification, designation or preference maybe. In essence the means if you have a severe unwanted hair problem caused Polycystic Ovaries Syndrome, another endocrine disorder or if your Transgendered my electrolysis treatments are geared to overcome the toughest cases with success.

Women with acute Poly-Cystic Ovaries and Transgendered patients call me and tell that their previous electrolysis and laser treatments have been failures. Their complaints detail the worst side of the profession. Some report that their chosen electrologist was neither professionally educated nor licensed and registered as an electrologist. Others state that the extent of their treatment was the less successful with flash electrolysis also known as Thermolysis, unsurprisingly, that their unwanted hair count has increased. Understandably, these patients are nervous because of past negative experiences with non-licensed quacks who claim they are Certified Professional Electrologists. Being the first and most experienced electrologist to use the computerized epilator exclusively, has enabled me to provide care second to no one, guaranteed. All Trans patients have a unique and complicated set of needs deriving from years of efforts to control an unwanted hair problem through standard shaving, waxing and tweezing, and the challenge of establishing and maintaining  a healthy hormonal balance. Understanding these hormanal complexities has enabled me to design and put into practice a detailed, methodical, systematic technique whereby I am able to permanently remove a beard with a possible 90% follicle kill rate and in less time than most electrologists. With a series of treatments adapted to their skin's needs, my patients have a beautiful, feminine, smooth, hair-free complexion with no scabbing, scarring or pitting. 

Too many electrologists fail to comprehend Galvanic Current let alone they do not take the time to understand its secret idiosyncrasies and is the most demanding treatment for an electrologist to perform. Galvanic Current in the profession is known as the Chemical Current and is the only true form of electrolysis the results in an Electrolytic Conversion Reaction. Whereas hair removal using Thermolysis only is a heat a reaction that electro-coagulates the papillae, cutting off the blood supply to the hair. The blend is the simultaneous use of the Galvanic and Thermolysis currents. However if you ask me what is the best treatment for white, black, coarse, tough, curved, distorted hairs and follicles that are resistant to Thermolysis, I have only one answer and that is straight up Galvanic Current.  Fact, in 1979 I was one of three licensed and registered electrologists in the Commonwealth of Massachusetts that knew how to perform Galvanic and Blend currents since electrolysis schools in Massachusetts only taught Thermolysis until 1984. For the record I learned to do Galvanic because my own unwanted hair problem was so resistant to Thermolysis, when I started galvanic treatments my progress was phenomenal and my unwanted hair problem was permanently removed. In the beginning Thermolysis is an excellent way to rapidly clear large areas but when you get down to the tough-cases that's when the seasoned professional electrologist steps in and uses computerized single needle Galvanic. It takes an educated experienced electrologist with expertise and know how to perform safe galvanic treatments that reduces the time you will need for treatment from 50% to 90%

The techniques that I have developed in order to meet the needs of the Transgendered-Population make a full transition a reality not a Barbie Doll fantasy. Every woman wants to be able to walk out her front door and feel confident about who she is and how she looks. Let's face it, we live in an appearance conscious culture and the incessant criticisms and consequent insecurities can take their toll. A bad unwanted facial hair problem is devastating to any woman's identity and self-esteem. For any and woman who experience extreme degrees of rejection and disparagement, living in one's skin is daily torture. I have treated many Transgendered women and I know for a fact that no amount of plastic surgery, or Industrial Grade make up will hide a heavy beard whether it is black, gray, red or even  translucent white. A close shave and a layer of heavy makeup lasts for two hours at best. After that the game is up as the beard lifts the makeup off the face and the shadow and stubble is back. A Trans-woman's comfort with being out and about in the world or work place is extremely limited. Here in the United States, we live in the world's most critical hair free society and so Electrolysis is the prerequisite for every Transgendered woman to be accepted socially for who she is inside!  Transgendered women who are my patients are able to fully realize their lives as women and no longer struggle with genetic ambiguities. They are freed from the hours in front of their mirrors applying quarter-inch thick layers of makeup to cover and hide the remaining beard because the beard is gone! Trans Women have to look real and be like any other women and that means having confidence in their appearance. For those with their doubts I welcome the opportunity to prove that true!

The case study presented here has been compiled with permission from the patient and in full compliance with professional criteria for confidentiality. Her name and any identifying details have been changed. The Photos used throughout have been selected with her approval. There are no air-brushed or doctored photographs here; preparing this case history did not require “Cyber Plastic Surgery' to create the visual effect of proper treatment.  The visuals are real and the timelines accurately reported. As this article is intended to serve both as a beacon of hope to the potential patient and an educational standard for members of the field of Electrology, relevant elaborations of technique and physiology will be made throughout this presentation.

Boston Electrolysis® Patient Profile Consultation & Evaluation:

Jonnie called me from her home in the Northwest in January of 2008.  I was not the first electrologist whom she had contacted to address her unwanted hair problem.  Jonnie had started treatments with an electrologist in her home area and as I proceeded through my normal phone intake questions for out of state patients it became clear that the practitioner was not well-trained and in all likelihood not registered and licensed. I heard the details that make me wince on behalf of my patient: excessive facial edema following treatments, excessive reddening of the skin a reaction clinically termed erythema, scabbing at the follicle entry, fortunately no weeping which is a sign of impending infection. Jonnie reported that her chin had swollen to the size of a baseball and that it had taken one month for her skin to heal from the trauma of the first treatment.

Many patients believe that these after-effects are normal consequences of the electrolysis process. It is a fair deduction to make. Patients can only draw their conclusions based on the norm for electrolysis results.  That unfortunate norm is determined by the numbers of practitioners who as a consequence of their abbreviated education and training are ignorant of the knowledge of histology, endocrinology, and practical techniques essential for healthy, effective results for their patients.  I often suspect that if electrolysis treatments were consistently and appropriately administered then the disastrous practice of laser hair treatment many never have claimed so many victims.  I do not use that term lightly.  In the days before the web and magazine pages were glazed over with laser ads, I would see only the effects of shaving, plucking and waxing on my patients, in recent years; however, I have seen a dramatic increase in patients whose hair and skin problems have been exacerbated by laser techniques. In large part as a result of this and other watchdog articles on this specious technique, laser treatment as a hair removal approach finally is coming under public scrutiny. This case history is a demonstrative argument in favor of electrolysis and its potential when expertly administered.

Boston Electrolysis® Patient History:

As with all my out-of-state patients, I followed up my initial phone conversation and basic treatment history with a request for three high resolution photographs of the areas that Jonnie wanted to have treated. Integral to an effective hair removal treatment is the electrologist's discernment of the specific needs of the patient's skin.  My understanding of my patients begins with that first photo views.



      


Jonnie is a fifty year old TS woman of Asian-Caucasian origin in excellent health. Her ethnicity and age present unique concerns for her treatment. Her skin is fair and fragile and prone to dryness but her hair growth was intense, heavy and concentrated on the upper lip and chin with a sparse growth on her cheeks and neck. The initial photographs revealed the damage that her years of shaving as well as her first electrolysis treatments had exacted on her skin.  The first two layers of skin, namely the Stratum Corneum and Stratum Lucidum, are composed of dead cells. Mother Nature's evolution designed the first two surface layers of skin to be dead to prevent deadly bacteria from invading our bodies. Live bacteria cannot penetrate dead tissue unless there is an open wound, scrape, cut or abrasion for it to enter. Shaving effectively scrapes those first two layers of skin off the face and exposes the live layers of skin known as the Malphigian and Corium layer to bacteria, which exposure often leads to acute Folliculitis, infected and inflamed ingrown hairs and pustules, and  worse, even a systemic infection of your skin. No educated, licensed electrologist will work on inflamed, infected skin or allow that a patient shave between treatments. Often shaving is the direct cause of Folliculitis and it makes unwanted hair problems worse by stimulating hair growth.

Boston Electrolysis Treatment Protocol:

My treatment protocol for all my patients but especially for my trademark Express Beard Removal© treatment employs a Blend Thermolysis Galvanic current treatments of the hair follicles. There are many Electrologists who, because either they are not educated in theoretical applications of Galvanic current or perhaps they have not invested in the needed quality technology, establish their treatment protocol using Thermolysis aka Short-Wave and Flash Electrology alone.  Based on my assessments of patients who were treated solely by Thermolysis and Flash-Electrology and by my subsequent results from my signature blended treatment, it is my opinion that practitioner's who are ignorant of or do not employ galvanic and blend treatments are impeding their patients' best outcomes.  In Jonnie's case, the hair per inch count had increased as a result of her former treatment.  I am accustomed to explaining to my patients the unfortunate fact that a portion of the work that I do is repair work making up for other practitioner's mistakes. However I warn all patients that total and exclusive use of the Flash-Electrology with its excessive high current leads to permanent skin damage and acute follicultis.

Key to my technique is a detail that patients take for granted when visiting an electrologist and shamefully it is an aspect of treatment where most electrologists fail, namely needle insertion. Very few people are aware that there are no nerve fibers in the follicle structure. Using precise exact deep insertions beyond the papillae into the blood supply (capillaries) insures maximum results with a virtually pain-free treatment depending on the location of the hairs. Furthermore, administering treatments in this manner induces electro-anesthesia (temporary loss of the skin sensitivity) where the treated area feels like it has been numbed with an injection of Novocain. This is my treatment of choice that I have developed for tough, resistant, severe unwanted facial hair problems such as deep-seated plucked, waxed, shaved and laser treated hairs stemming from Polycystic Ovarian Syndrome and other forms hyper-trichosis. An untrained self-professed electrologist treats all hairs as equal and does so ineffectively at that by failing to properly perform the most basic of techniques.  Proper needle insertion allows me to treat the hair follicle aggressively without causing my patient pain and discomfort. With the exception of the sensitivity of the upper lip, which area I treat in alternation with outlying areas so as to allow the patient respite, patients find my electrolysis treatments are quite comfortable and relaxed experiences.  Jonnie even caught a few cat naps during treatment.

 

Boston Electrolysis® Treatment Series:

After our phone intake and my photo based assessment, Jonnie and I agreed upon a treatment plan for her unwanted hair. In early February, Jonnie flew down from her home in the Northwest with arrangements to stay in the greater Phoenix area for one week. When performing a complete beard clearing I begin with an initial shorter term treatment. My objectives for the first session are to establish a healthy rapport with the patient; to answer her questions and address her concerns that may have arisen for her since our phone intake; to confirm my diagnosis of the patient's skin and. Establishing a healthy rapport with the patient is essential and often where other practitioners first fail.  Defining the expectations for treatment and the boundaries of the professional relationship puts a patient at ease, initiates trust, and confirms your professional commitment. A first session can be anxiety-provoking if the patient has had previous negative experiences with hair removal. A short one-hour treatment is sufficient time for me to began to clear outlying hairs and to define the treatment area but not so much time that it will prove stressful to the patient. Additionally, a short first treatment serves as a skin response trial for the patient. She can rest assured that the deformation and pain of her previous treatments is not the standard of outcomes. In twenty-four hours she can return to my office assured and ready for the intensive work.  For the eager patient on the verge of tearing the hairs out herself, my insistence on starting slowly and methodically highlights my experience and sets me apart as an electrologist who is concerned not with fast and temporary outcomes but with true results.  Good methods yield the healthy, smooth complexion that patients hope for and have a right to expect.

Boston Electrolysis® Day One:

Jonnie and I met for our one-hour first session on Monday February 6, 2008. She was both eager to get the treatments underway but also a bit apprehensive given her previous experience.  As noted above, Jonnie's beard growth was heaviest on the upper lip, chin and neck in addition her skin showed the thickening and callousness that results from years of shaving. As seen in the photo, a number of follicles on the under chin had ingrown hairs and the overall condition of her skin was distressed. The first session lasted for one hour during which time I administered a series of galvanic follicle treatments beginning with outlying hairs on the neck and cheeks.  In that session I treated a total of 86 hairs. Jonnie's skin responded with expected mild erythema with minimal edema as a result of increased blood flow viscous fluids to the treated area. Jonnie expressed no discomfort during the treatment.



Boston Electrolysis® Day Two
:

Jonnie reported that the initial erythema from the treatment had subsided within a few hours of the first treatment a fact indicative that I had exacted no tissue damage as a result of the initial treatment. On the day scheduled for her second treatment, Jonnie was comfortable with her outcomes and ready to move forward with the intensive treatment. On the second day, the treatment session lasted for seven hours and thirty minutes during which time 720 hairs were treated.  Given our shared objectives, Jonnie's skin needs and her comfort level, I utilized galvanic current for the entire treatment series.  Whether to do this is a decision made on a patient by patient basis. In Jonnie's case, this optimal methodology was possible.  This is not always the case. If the patient's hair density is too intense or the skin is too inflamed from shaving, plucking and waxing I will opt to use short-wave treatments for a few rapid clearings especially if the hairs are too dense meaning the surface area to be treated has more or an equal amount of hair than skin. Dynamic progress is achieved with high accuracy insertions and rapid clearing that will require less treatment time that is verified with a computer hair count for every area on the face and neck that is treated. Fact with Short-Wave my hourly hair count is 400 to 900 hairs per hour depending on the hair and skin condition and the total surface area to be treated.

Typically it is after the second or third treatment that I will start galvanic treatments using from 6, 8 and up to 15 milliamperes and up depending upon the condition and thickness of the patient's skin and hair type.  The initial galvanic bubbling of the skin subsides after the beginning of the treatments and is gone by the time patients get home to do their prescribed Epsom Salt Soaks.

Jonnie's skin responded well throughout the treatment.  Her hair density permitted the sole use of galvanic current administered at 9-14 milliamperes for an average of 20, 30 and 40 seconds per hair with second and even third rounds of current causing the follicle to become completely penetrated by and permeated with sodium hydroxide. The long term success of treatment was evident in the state of the hairs that were removed.  The hairs released easily from the follicle with no resistance and were coated from root to tip with a thin whitish translucent layer of sodium hydroxide. In the time that we had scheduled, I cleared the concentration of hair on her chin and cleared 100% of her upper lip, a goal that we set together.

Most of the Galvanic current's ability to destroy the hair growing cells is caused by the electrolytic action and the electrodessication (drying out the entire hair) and electrolytic-chemical decomposition that results in the break-down of the entire structure of the active hair growing follicle. If your electrologist is educated and well versed in Histology and wise to ways of practical experience using Galvanic current and other combined modes the electrologist can insert the probe into root structure and well past the active growing hair and into the germinative papillae and treat the coiled bed hair known as “baby anagen” and directly into the capillaries couplers known as the blood supply that nourishes each and every hair of the follicle.  A deeper insertion can only be applied to the chin and neck and others areas of skin with a satisfactory thickness of adipose tissue in good supply inserting the probe this deep treats the coiled bed hair attached to the papillae of the active growing hair that an electrologists occasionally sees but many electrologists do not comprehend the origin of the bed hair. The reason most electrologists do not see the bed hair is because the microscopic cellular stage of hair growth is not visible to the naked eye. It can be seen, however, under a powerful microscope. Additionally after two to four weeks of hair growth with 4.25 magnifications, most electrologists often see the bed hair without giving a second thought as to where did this hair come from, because most electrologists are only concerned treating the hair that protrudes from the skin. There are times when electrologists remove the main hair and the smaller hair comes out attached to the main hair which means that the hair will never regenerate.

The elimination of hair growth potential at the microscopic cellular or germinative stage can only be accomplished with Galvanic current. It is the chemical reaction of Thermolysis induced by applying galvanic current to the follicle that is responsible for the elimination of the follicles' ability to generate hair. Three seconds of Galvanic current catalyzes a reaction within the follicle causing the follicle to produce lye, a highly caustic compound that destroys the undifferentiated cells found slightly higher up in the follicle, called stem cells. These are the cells responsible for new hair growth. Without them, the follicle is permanently out of commission. However all the nourishment for the hair and replacements hairs come from base of the follicle in the Germinative Chamber. Additionally, the Galvanic current is available and able to successfully treat curved and distorted follicles due to its spreading action. The ultimate benefit of Galvanic Current is the Lye a.k.a. Sodium Hydroxide remains in the follicle and after removing the hair so in essence the follicle structure is still being broke down until the skin excretes it.  The initial galvanic bubbling of the skin subsides from the beginning of the treatments and is gone by the time patients get home to do their prescribed Epsom Salt Soaks. Applied correctly there is no next day scabbing or excess edema although on first time treatments of this nature some galvanic treatments have slight edema for a day or so. However on second galvanic treatments there is none or no noticeable edema or scabbing on the day after treatment.

Boston Electrolysis® Day Three

Jonnie was truly pleased with her results following the first intensive session on that second day of treatment. None of the nightmarish results were playing out on her complexion.  On the third day, I continued with the use of Galvanic current. The primary development of my Trade Secret Technique utilizes my firm three-way stretch using a highly accurate swift deep back-handed insertions the with computerized current timing set from 10 to 20-30 and 40 second applications of galvanic current per hair. The computerized epilator is programmed to administer a second round of current when the probe is not removed from the follicle.  If the probe remains inserted, the epilator will automatically deliver a second round of current after three seconds with the option for a third or fourth round if needed. This allows me to perform, maximize and insure the patient's comfort for each and every follicle during electrolysis treatments while achieving outstanding results with each and every hair. Further, this is why I invariably insist that an electrologist invest in a computerized epilator. The bottom line is always quality of care for the patient.  On Day Three, I continued the work on Jonnie's chin and upper lip as well as removed the number of ingrown hairs on her chin just below the jaw bone. The session lasted for three hours during which I cleared 463 hairs.

Boston Electrolysis® Day Four

On day four the effects of treatment showed their promise.  The preponderance hairs had been treated and Jonnie's delicate skin was healing and showing the first signs of being restored to its smooth healthy state. The session lasted for one hour and fifteen minutes during which 137 minor hairs were treated.



I continued with treating the upper lip and chin hairs just above the bone.  Attending to the sensitivity of the upper lip and given that the preponderance of hairs had been cleared, the session lasted for one hour and fifteen minutes and 137 hairs were treated with 20, 30 and 40 seconds of galvanic current from 8 to 15 milliamperes.

Boston Electrolysis® Day Five

On days five and six, Jonnie enjoyed her emerging true self and rested. I worked with my regularly scheduled clients of the time.

Boston Electrolysis® Day Six

Day six was the wrap up session for Jonnie's initial Express Beard Removal® treatment series. In a session lasting one hour and forty-five minutes I cleared all the remaining peripheral hairs on her face, a process I call microscopic grooming, treating a total of 157 hairs. Jonnie was extremely happy with the results and eager to return home to share her success with friends. I took the time to explain to her what she could expect in the upcoming months.  The condition of the hairs and the fundamentals of my technique were grounds for expecting a maximum kill rate for the follicles. Given the life cycle of a hair, however, meant that Jonnie and any patient can expect some new growth following treatment.  With many electrologists, new growth is indicative of a failure to kill the hair in the germinative stage.  With my intensive treatments, new growth is more typically the emergence of hairs that were in the subcutaneous Baby Anagen stage and therefore inaccessible at the time of the treatment.  I adamantly insisted on my two principles of inter care. Firstly, that Jonnie not shave, pluck or tweeze any hairs that emerge following treatment and secondly, that Jonnie maintain her healthy hormone levels.

From my professional observations, a TS who does not take full responsibility for insuring that her body is hormonally balanced (an imbalance shows up with a dominance of androgens) will slow if not reverse the progress of electrolysis treatments. With the right hormonal balance the progress of electrolysis treatments can be enhanced not to mention it is a known fact that when hormones are at the optimum levels patients feel fit and emotionally well. The responsibility for maintaining full knowledge about a patient's hormonal levels, however, does not rest solely on the shoulders of the patient.  As a practitioner who served as a guest lecturer at Harvard Medical School from 1983 to 1987 and presented on Transsexualism, Human Sexuality, Endocrinology and Electrolysis I believe that is so essential as to be an ethical imperative for an electrologist to understand the endocrinological needs of patients. 

Most electrologists are not aware that DNA needs certain vital chemicals AKA hormones for it to function properly. Another fact that most people do not know that the human body can produce up to 1200 follicles per square inch of skin however not all of them will grow hair unless you have a high androgen levels or extreme "Hyper-Trichosis." Too many Electrologists go on the assumption a hair is hair and ignore the effects of hormones and without one doubt estrogen is a vital hormone that lowers and suppresses androgen levels which in turn decreases your Luteinizing Hormones. Fact estrogens will retard the masculine affects of androgens however it will not cause the hairs that you have developed to fall out. However estrogens do decrease and negate the element of the androgens that triggers growth and strengthens hair and that make your treatments much more effective in treating and eliminating them.

Furthermore in certain cases estrogen will prevent new hair growth especially when TS'S who have taken estrogen for two years and is postoperative. All this being said, a coarse hair is a coarse hair. The only way to permanently remove that hair is with electrolysis. Additionally estrogen reduces the LH levels known as (Luteinizing Hormones) in your system increasing the effectiveness of electrology and electrolysis. Even if the "Luteinizing Hormones" have affected the texture of the hair, it is the number of hairs that I am concerned about that need to be eliminated.

Many TS'S merely take their estrogen as prescribed and do not know to be concerned about their overall hormone profile. They then become quite concerned when their electrolysis treatments hit an unending plateau of no progress. I require all TS patients to know what their current levels are and to provide me with a copy of their prescribed hormone levels. It would be tantamount to thievery on my part if I did not actively engage my patients in a dialogue about the potential instigation of new hair growth consequent to a rise in androgens. Every time I have suggested to a TS patient that her hormone levels are too low and need to be adjusted, once the dosage is corrected, her progress improves.  In the beginning of hormone therapy a client's levels need to be monitored to attain the best results that vary from moderate to excellent progress. It is imperative for the TS patient to check her hormone levels every three months while beginning and undergoing electrolysis treatments.  Jonnie was fully engaged in monitoring her levels and assured me that she would continue to do so.

The hair shaft was dull black and came out with a puffy glistning intact root sheath and germinative papilla from the base of the follicle. A hair treated by Galvanic Current easily releases clean, moist and 100% intact from the base of the folliclle is the hallmark of maximum follicle destruction by Galvanic Current. Without one doubt Galvanic current causes the complete Electro-Dessication and Electro-Decompostion of the entire follicular struture from the base of the follicle to the very top. Fact I learned how to do Gavanic-Treatments because my treatment progress was slow and you get tired of unedning repetitive short-wave treatments. However I am believer in all three treatment modes, Short-Wave, Gavanic and Blend and it's my professional opinion that an electrologist who resticts themselves to Short-Wave treatment lacks the training, patience and the practical skill needed to peform expert Galvanic Treatments.

Resolution:

Jonnie's Express Beard Removal treatment is a success.  With two series of treatments totaling 8 sessions with 20 hours and 2001 hairs were treated and eliminated. Jonnie is now a woman fully confident of her appearance and thoroughly enjoying the freedom from anxiety that so marked her years before treatment.  Her skin always held the potential for the remarkably smooth  marble-like complexion that she now enjoys.
